This application has 24 questions that will require thoughtful responses, such as your statement on "How do you know your sins are forgiven?" Since church membership is a testimony of true spiritual conversion, it is important that you are assured of your own salvation. The application consists of three parts: General Information, your Faith Story, and your Membership Commitment. It could take 20 minutes to 1 hour to fill this out depending on how thoughtful and detailed you want to be in your responses.
